Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorized members of staff. If you wish to obtain a copy of your personal health record, or part thereof, please ask the receptionist or doctor. You will then be asked to fill in a form regarding this.

PRESCRIPTIONS, REFERRALS, AND MEDICAL CERTIFICATES
Patients need to consult with the doctor prior to prescriptions or referrals being issued. We will not, for medico-legal reasons, backdate referrals. Please check that you have a valid referral with your specialists before you see them. Medical certificates will only be issued with consultations.
RESULTS
In most cases, results may be given over the phone by way of a Telehealth consult. Reception will not be able to provide any results, as an appointment with the doctor is needed. When an investigation is performed, such as a scan or blood test, patients need to make an appointment through HotDoc or at reception to discuss the results. If normal, Pap smear results will be mailed to your preferred address.
